There are several different techniques of rhinoplasty surgery, however, only two basic methods have become predominant. The two main procedures of rhinoplasty surgery include:

  • Open Rhinoplasty
  • Close Rhinoplasty

Let's look at each of these types of rhinoplasty in more detail.

In open rhinoplasty, the surgical procedure involves a small unseen incision made between the nostrils in the nasal septum. This method provides a comprehensive view of the internal nasal structures through the small folding of the skin. It is used for more complex cosmetic rhinoplasty or functional rhinoplasty procedures (septoplasty or conchotomy). It provides the surgeon with optimal visualization of the internal structures, helping him or her to accurately adjust the nose reshaping, even in difficult cases such as after an injury. In addition, it allows for easier stabilization after surgery, reducing the risk of future nasal complications. Although the scar is very small the surgery tends to be more time consuming and with a longer recovery period compared to closed rhinoplasty.

Closed rhinoplasty is the most traditional method and is performed intranasally, i.e. the incisions are made inside the nostrils, so there is no risk of a visible external scar. This method is usually recommended when the patient wishes to have minor corrections such as reshaping of the nasal septum. Although it has the advantage that the procedure takes less time and has a shorter recovery time, the limited visibility of the surgeon during the procedure may limit the extent of the modifications that can be achieved. This technique can also compromise the structural integrity of the nose, increasing the likelihood of future stability problems.

Augmentation rhinoplasty

Augmentation rhinoplasty focuses on strengthening the nasal structure, often using implants or grafts to augment and improve the nasal profile. Whether performed with either an open or closed technique, this method aims to create a more balanced and harmonious nasal appearance, addressing concerns associated with a flat nasal bridge or inadequate nasal projection.

Reductive rhinoplasty

Reduction rhinoplasty, on the other hand, aims to address issues associated with a prominent or oversized nose. Rhinoplasty of the tip

Rhinoplasty and genioplasty

Diaphragm plasty and conchotomy

Problems involving the septum and nasal niches often lead to obstruction of nasal airflow. The septum, a thin wall of bone and cartilage that separates the two sides of the nose, is often crooked and unsymmetrical, which obstructs breathing and causes symptoms such as snoring, sleep apnea, headaches, nosebleeds or infections. Also the nasal niches that warm, filter and moisten the air before it enters the lungs, when they become hypertrophied cause dry mouth, altered sense of smell, runny nose or even facial pain.

Diaphragmoplasty and conchotomy both are effective procedures to correct these problems. Diaphragmoplasty aims to straighten the crooked septum by restoring normal airflow dynamics, while conchotomy addresses the hypertrophy of the nasal cavities, restoring their functionality.

How long is the recovery after rhinoplasty?

The plastic surgeon will provide you with details of the visit schedule.

  • Stitches: They are dissolved or removed by the doctor in the first week.
  • Protection: The nose is covered with a bandage and splint to stabilize the new shape, which is removed in 6 – 7 days after surgery.
  • Patch change: This is a gauze that collects possible blood. The doctor will give you instructions to change it yourself if necessary.
  • Patch inside the nose: Reduces bleeding and swelling.

How long is the recovery after rhinoplasty?

Recovery time after rhinoplasty surgery depends on the severity of the surgery and your individual healing process. However, in general, you can expect to return to your daily activities within a week or two after open rhinoplasty and within a few days after closed rhinoplasty.

Open rhinoplasty

Immediately after surgery:

Patients experience disorientation on awakening. A responsible person is recommended to drive you home and stay overnight with you.

Rest and adherence to the dosage of prescribed medications is important.

1-3 days after surgery:

You will be able to remove the gauze from the inside of your nose.

1 week after surgery:

Use cold compresses and anti-inflammatory medications to relieve swelling, bruising, bleeding and congestion.

Strict avoidance of actions that put pressure on the nose, such as blowing the nose, pressing on the surgical site or wearing glasses.

Light movements indoors are encouraged. However, vigorous exercise should be avoided.

2 weeks after surgery:

Possible return to work or routine activities, depending on the type and extent of surgery and the nature of the work.

Gradual resumption of light activities such as short walks, housework and driving if you are comfortable and not taking painkillers.

2 – 4 weeks after surgery:

Resuming exercise is generally safe after four weeks, but caution is recommended to avoid injury, especially in activities where there is physical contact such as football or basketball.

Closed rhinoplasty:

1-2 days after surgery

You will be able to remove the gauze or splints. You can expect to experience some swelling and bruising, but usually the symptoms are less severe than in open rhinoplasty.

1-2 weeks after surgery

You will be able to resume most of your daily activities.

2-3 weeks after surgery

The swelling and bruising should have subsided and you should be able to see the final results of your rhinoplasty. As with open rhinoplasty, you should avoid strenuous physical activity during your recovery.

What are the risks of complications in rhinoplasty?

In general, rhinoplasty has proven to be a safe surgical procedure. The risks of rhinoplasty are small, transient and serious complications are rare.

Common transient effects after surgery: 

  • Bruising and swelling: Bruising of the nose and eyes, swelling may peak before subsiding in 3 to 4 weeks.
  • Pain & discomfort: nose pain, possible headaches.
